Re: In Discussions for a merger with Longrove Medical Practice
We seek your views on our near future plans to merge with Longrove Surgery. Longrove Surgery have moved into our building and we work from the same floor and are both working with the same aim which is to provide an excellent healthcare service which is of high quality standard it makes sense to work jointly. There are several benefits:
- There will be more doctors available. There will be 5 clinical partners plus several salaried doctors and trainee doctors.
- There will be a much larger non-clinical team.
- There will be a dedicated pharmacist, a paramedic and MSK Practitioners together with more nurses, nurse practitioners and Health Care Assistants available.
- More appointments will be offered to patients
- One system will be introduced instead of two practices working in different ways.
We hope that you agree with our views. We are planning to merge on 1st of January 2021.

Patient Participation Group
We would like to know how we can improve our service to you and how you perceive our surgery and staff. Membership is voluntary and we promise not to take up too much of your time. We would like to encourage you to have a say in how we manage our service.
To help us with this we will ask the Members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will also contact you via telephone or email to ask for feedback. The meeting minutes and survey results will be displayed on this website so you can see what improvements we are making within the surgery.
Vale Drive Medical Practice
Providing primary care for Vale Drive and surrounding areas.
This website provides many online facilities, including appointment booking and cancellation, registration, prescription ordering, and much more. It also contains a vast range of health information and resources, along with practical information on visiting the surgery. We hope you find the website useful and informative.
We now offer our patients the use of a secure website on the Internet to:
- View and request repeat medication
- View, book, and cancel appointments
- View information from your care record.
Please come to the surgery and register for this service. You will need to bring in photo ID and a proof of your current address.
Named Accountable GP
At Vale Drive Medical Practice your named accountable GP is Dr Raithatha. From 1st April 2017 Dr Raithatha will take lead responsibility for the coordination of all services and ensure that they are delivered to the patients based on his clinical judgement. However, it does not mean that he is responsible to see you when you book an appointment. You will be seen by either Dr Raithatha or our Salaried Doctor or Locums.
Access to Medical Records
You have a right to know what information we hold about you. If you would like to see your records please contact the Practice for details on how to go about this. Please note that there is a charge to access medical records.
The practice will send out automated reminders regarding pre-booked appointments via your mobile telephone so please keep us informed of your latest mobile number. If you change your mobile number or landline then please notify us. If you DO NOT wish us to text you to remind about your pre-booked appointment or a reminder of various promotion of health checks that you may be due to then please click here to download the Dissent Form and return the completed form to the practice.You can either return it in person, by post or by email to Vmedical@nhs.net
We will assume that if you do not complete the communication Dissent Form then you are happy for us to text you.
